Pre-contractual information for policyholders

pursuant to Articles 45, 45a and 45b of the Swiss Federal Act on the Supervision of Insurance Companies (ISA; SR 961.01)

1. Purpose of this information

This information from Callenium GmbH (the “Company”) is prepared pursuant to Article 45 et seq. ISA. It is made available to the user before any insurance advisory or intermediation activity begins and, in any event, at the time of first contact. This information does not constitute a contractual proposal, an insurance offer or a brokerage mandate. The advisory and intermediation mandate is governed by a separate contractual document.

2. Identity and address of the Company (Art. 45 para. 1 let. a ISA)

“CassaOK” is a service operated by: Callenium GmbH Bahnhofstrasse 15 8253 Diessenhofen CassaOK is not a separate legal entity. Every legal relationship arising from use of the comparison service, advisory services and intermediation is established with the Company.

3. Intermediary status and FINMA register entry (Art. 45 para. 1 let. b ISA)

The Company’s activities – including preparing a personalised premium comparison based on the user’s individual data, sending a personalised report and providing subsequent advice and intermediation – constitute insurance intermediation within the meaning of Art. 40 ISA and Art. 19a HIA in relation to compulsory health insurance. The Company acts as an independent insurance intermediary within the meaning of Art. 40 para. 2 ISA. It is not legally, economically or otherwise tied to any insurance company and does not represent any insurer. The Company acts in the policyholder’s interest. Entry in the public FINMA register of independent insurance intermediaries: no. F01253815 The Company holds no equity interests in insurance companies and no insurance company holds equity interests in the Company. No member of the Company’s governing bodies or employee holds a management position at an insurance company. 4. Liability and financial security (Art. 45 para. 1 let. d ISA)

The company responsible for claims arising from errors, negligence or incorrect information in the course of intermediation is Callenium GmbH. Information about the training and continuing education of Callenium GmbH employees is available directly from us: Callenium GmbH Bahnhofstrasse 15 8253 Diessenhofen Liability for the technical operation of the comparison service, the accuracy of data entered by the user and service availability is governed by the General Terms of Use. Article 100 para. 1 CO remains applicable in all cases; it prohibits advance exclusion of liability for intent or gross negligence.

5. Remuneration received by the Company (Art. 45b ISA)

The service is free of charge for the user. The Company is remunerated by the insurance companies with which it cooperates. The user is expressly informed of this by this statement. The client is informed and expressly waives the right to be informed by Callenium of the exact amount of commissions, fees and additional remuneration received, and waives the right to request disclosure of the same from Callenium. This also applies to commissions, fees and remuneration paid after the end of the contract.

6. Updating this information

If the information referred to in Art. 45 para. 1 ISA changes, the Company will inform clients at the time of the next contact.
